After your consultation, you will receive a client intake form via email, which you will be providing personal and vital information about your surgery.
Please include as much information as possible in regard to your procedure. This will better prepare our team to provide you with exceptional aftercare.
Once you have completed your intake form, your contract will be sent via email, which will detail your deposit and total payment.
